Using CitizensHealth is simple and convenient:
- Make an Appointment.
- To locate a CitizensHealth discount provider, please see the Provider Directory
included with your member materials, call CitizensHealth Member Care Services
toll-free at
1-800-214-5697, or visit the
Find a Provider section of this website.
- Call the provider’s office in advance to make an appointment. (This is
not necessary for visits to the pharmacy).
- If your provider does not recognize the CitizensHealth name,
you can inform them that they participate in CitizensHealth through their
discount network affiliation. The participating discount networks appear
on your Membership Card.
- Always remember to keep your appointments!
- Take your CitizensHealth Membership Card with your to your appointment
- At your appointment, show your Membership Card. If you are
going to a medical doctor appointment (only if your plan includes medical
care), bring along the enclosed Letter to the Provider.
- Enjoy your Savings!
- You will need to pay the full, discounted cost of your services
at the time of your appointment.
